You are required to complete this form if you would like the Trustees to consider receiving benefits from another Scheme Administrator. We will write to the existing Trustees or Scheme Administrator to obtain the details required.

This form gives us authority to deal with your existing Trustee or Scheme Administrators to attend to the transfer to the Prophecy SIPP.
